[Groff] Questions about the MM macro package

Hello all,

I  am learning the MM macro package and have met a couple of
problems I couldn't solve after reading the man page:

 (1)   Is it possible to  omit  the  standard  page  headers
       without using .LT?

 (2)   My  test  document  begins with a call to the .PGFORM
       macro, setting a different page width, but the  stan-
       dart header on the first page is still centered as if
       the page width was  default.  Is  it  the  right  be-
       haviour?
