I have exactly the same problem using geoserver 2.12.1. I can connect with the LDAP user, I also see the LDAP groups as geoserver Roles. But the connection between LDAP users and LDAP groups seems to be lost.
What I have already found is that it is has something to do with the LDAP tree I'm using. When adding another tree with nearly the same configuration, then it works.
