Thanks everyone for help. I found and solved the problem. It was Tom's
suggestion to look at trailing slashes. In my httpd.conf virtual host
section I included a trailing slash on the URL. Dropping the slash solved
the problem.
For some reason with WMS it wasn't a problem but GeoWebCache didn't like
the slash.
