Thanks for the info.
So I've just removed the "diskquota.index.db" file and restarted Geoserver,
seems to be ok now. Is this all I can/should do?

Seems to be an issue with H2:
https://code.google.com/p/h2database/issues/detail?id=88
GeoServer is using an old version of H2 because of difficulties in upgrading it
(we'd have to upgrade all of its
users at the same time, which would require testing a few subsystems).
I guess we can try to raise the priority of the upgrade, but without funding
addressed to it it's still
going to fall in developers spare time among a lot of other things.
