Also, assuming it's KDE then also run pavucontrol-qt (or whatever it's called on your system) to ensure you see the application generating audio is actually doing what you think it is - there's an app-by-app set of VU meters showing where KDE is sending audio in terms of which hardware output. I.e. - KDE can send audio from different apps to one or more hardware devices at the same time. pavucontrol should allow you to see how this is happening at any time. I have it in my favorites.
