Upstream means someone above Gentoo issued a release. Think of it this way - you have Gentoo who puts the packages together in ebuilds, etc. but they use someone else's package. For example, xfce is released by the xfce people - Gentoo just does an ebuild for it and makes it available to us. Think of it as a stream flowing with the people like xfce who make their packages as being upstream from Gentoo. When someone upstream (like xfce) issues a new release Gentoo has to include it in portage.
