It is a simple legal paper saying that you have the legal right to contribute stuff to the ASF. You should also consider to get your employer to sign a CCLA [3], which basically says that the company knows of these activities and that they agree that you do what you do. For some jurisdictions that would not be necessary for 'off working hours' contributions, but I have heard that some States the employer owns any output you produce, 24/7. Better safe than sorry, so check with your employer. It is probably easiest to have the forms sent to the Secretary, fax number and address is on the form.
