Doesn't reproduce with 4.0.4:

37260.C: In function ‘int main()’:
37260.C:8: error: ‘PTHREAD_ONCE_INITIALIZER’ was not declared in this scope
37260.C:9: error: initializer for ‘pthread_once_t’ must be brace-enclosed
