Note that the FreeDOS format runs but creates something that Windows
doesn't recognize at all.

I suspect the CHS  0-1-1 setting.

Me? Nope, I don't engage in kernel hackery unless driven to it while tracking down an external bug.

My flash drive boots to the A: drive, do you have that booting option? It's USB ZIP selection here. Just uses an basic boot loader image on the flash drive, though.
