2) No matter you think that your find fix the issue since the start of this 
discussion, please, i recommend:
- a) you to open a ticket on ticket tab at "SourceForge FluidSynth Ticket tab" 
(you need  to register an account on sourceforge).
- b) give a name to the ticket for example  "[Audio Driver] no effects applied 
using multiple stereo channels with Jack"
- c) gives a short comment on your find and a link to the start of this 
discussion.
Doing this manner is the only way to maintain source code coherent over future 
version.
 
Just last questions: 
Do you hear that the Voice MIDI Channel mapping to multiple audio ouput number 
works according the previous explanations ?
Do you hear Reverb on output 0 only ?
