I fixed it - it wasn't the Window Switcher settings that controlled which 
windows were placed at the top of the Alt+Tab list.  I went to:

 Settings/Windows/Window List Menu/Sort Order

and selected 'Most Recently Used'.

Previously it was set at 'None', which I think is the default.
