That was indeed what I meant.  For example, the hicolor index.theme referenced 
512x512/apps but that directory was not under the hicolor theme.  This was also 
the case for some of the installed themes in ~/.icons, and it were these that 
didn't show up in Settings > Application Theme > Icons.

Unfortunately fixing this didn't solve anything regarding showing application 
icons, so in the end I gave up and just linked the /usr/share/pixmaps to 
/usr/share/icons/hicolor/48x48/apps, and now at least I get icons for my 
applications.
