I have always seen this in association with itask-ng.  If I stop using
itask, I don't have ghost windows.  I have seen this for several months now,
at least two revisions of itask-ng.

Another incredibly reliable way to generate them is to use thunderbird and
compose email.  It always seems to create a ghost.  Firefox save dialogs
usually work too.  Based on the other applications I run, I think it is
somehow related to transient windows but I have no proof.

Restarting almost always crashes e as well, which would lead me to think we
have a pointer not getting cleared correctly.  I have followed the usual
steps of compiling with debug and attaching gdb after the crash, but it
never seemed to indicate any real problem.  Another really good indication
that a pointer isn't pointing where it should be.
