MdeModulePkg: CpuExceptionHandlerLib: Define X86_CPU_INTERRUPT_NUM
There are currently many definitions for the number of interrupts
for X86 processors. Centralize this definition in
CpuExceptionHandlerLib.

UefiCpuPkg: Set up 256 IDT Descs in SEC/PEI/SMM
Currently, SEC, PEI, and SMM only set up 32 - 34 IDT
descs. This means that any exceptions received above
that number will cause a #GP fault.
This commit updates these environments to follow DXE
and set up the IDT with the architecturally defined
256 entries, each of which point to the common
exception handler.
This does two things:
- Has well defined behavior for receiving exceptions in
these environments instead of relying on a #GP fault to
occur.
- Allows the stack cookie interrupt (0x42) to be routed
correctly instead of appearing as a #GP fault.

MdeModulePkg: DxeIpl IA32: Use 256 IDT Entries
When IA32 DxeIpl is setting up the IDT, it should populate
256 entries, the architectural size, so that exceptions are
handled in a defined way, going to the common exception handler.
This includes the stack cookie exception.

OvmfPkg: SEC/SMM: Use 256 IDT Descriptors
This follows the update in UefiCpuPkg to use
256 IDT descriptors in SEC and SMM. This is
done to:
- Have a defined action when exceptions are
taken (use the common exception handler)
- Allow processing the stack cookie exception
gracefully instead of generating a #GP fault.

UefiCpuPkg: Converge SEC/PEI and SMM/DXE Exception Handler ASM
Now that the SEC/PEI asm is doing 256 IDT descriptors, the only
differences are CET and XCODE support between it and the
SMM/DXE version of the assembly.
This commit merges the two files into one. XCODE support must be
handled differently between the two because in SEC/PEI we have
completely R+X images, so we can execute from the data section.
In SMM/DXE we do not have executable .data sections, but we do
have writeable .text sections (initially), so this code patches
up the .text section. This is the same behavior as before, only
merged into a single file.
