Remove dead byte-swapping code in dyninstAPI codegen (#1697)
* Remove insnCodeGen::ptrAndInc
It was added by 311466419 in 2015 as commented-out code.
* Remove use of endian_mismatch macro
Its use was removed by 44507e6b0 in 2013.
* Remove insnPtr
It was only used by ptrAndInc.
