Something that's been floating around in my head for a while is that the mi shadow module isn't particularly well suited for page flipping because it only updates the second buffer at regular intervals (every 1/50 second AFAIK). I finally got around to trying the XFree86 shadowfb module instead, and with
http://penguinppc.org/~daenzer/DRI/radeon-pageflip-shadowfb.diff I haven't seen any artifacts yet with WindowMaker (which rarely showed artifacts even before) or metacity (which very often showed artifacts before).
