It also adds
- constexpr default ctor and ctor from nullptr
- implicit ctor from nullptr (but explicit from T*)
- qintrusivesharedpointer.h doesn't transitively include the world
(I think I can even drop the <qatomic.h> and <qhashfunctions.h> - if we
can depend on proper two-stage name lookup these days :)
