I'm thinking that maybe your start-info parameter should just be <start- info="text/xml"> without the charset. The XOP spec says: <quote> The "start-info" parameter of the package's media type MUST contain the content type associated with the content's XML serialization. (i.e. it will contain the same value as the "type" parameter of the root part). </quote> i.e.i.e. not the same as the Content-Type HTTP header. The soap MTOM submission shows an example XML the supports this - see Table 2 of <http://www.w3.org/Submission/soap11mtom10/>.
