I been spending some time looking into this. It appears that the problem was
triggered by some invalid request headers being passed to ISAPI.

I have been unable to recreate this on Windows 2008 or 2016. My conclusion,
therefore, is that somewhere between 2012 and now, IIS tightened up its header
parsing.

I am therefore resolving this bug as WORKSFORME. If anyone is still
experiencing this issue with:
- a fully patched, currently supported version of Windows; and
- the latest JK 1.2.x release

then please feel free to re-open this issue and provide the steps to reproduce.
