It appears that the problem was triggered by some invalid request headers being passed to ISAPI. I have been unable to recreate this on Windows 2008 or 2016. My conclusion, therefore, is that somewhere between 2012 and now, IIS tightened up its header parsing. I am therefore resolving this bug as WORKSFORME.
