    Pull request with a number of minor updates to documentation, arising from

    Update to /guide/ops/locations/index.md to simplify text by moving some 
detail to sub-page.
    
    Move section "AWS VPC issues which may affect users with older AWS 
accounts" to its own
    page.  This was one of the observations from the First Encounter of the 
Belfast team.

    Tidying description of adding to and deleting from the catalog.
    
    Fix the URL example for deleting an application to use .../application 
instead of .../entity (actually both of them work, but .../application is
