Yes, this appears to be the case.
IIRC, they do but they are some place where only one adresse will be
printed, the first of the list, not the one that matches the current
email.
Yes, the email address in the subject line or first in list is
displayed, without relation to the actual email used. One can consider
this a bug, but certainly a valid feature request...
