What I'm curious is about on step 5, did the UI move back automatically to HDMI or did you need to do it manually.

That's when, I believe, things went wrong. At that point, the selection was moved automatically from the HDMI output to the analog output. When I now reselect the HDMI output, the HDMI output is selected in the left pane, but the right pane UI is not updated - the profile box still says "Analog stereo duplex" and clicking "Test sound" it says "Speaker testing for Analog output".
