Is this a laptop or a desktop? The monitors.xml.txt above is showing only an HP 23xi monitor connected by HDMI, but at very low resolution and refresh rate. So assuming that's the only screen you are using, most likely it's a poor quality HDMI connection and I would recommend getting a higher grade HDMI cable. After you reboot with a higher grade HDMI cable it is likely that more screen resolution and refresh rate options will be available.
