Repro LO 7.3+. This is Legacy Check Box Form Field in MSO. This DOC check box doesn't work in MSO 2016, something is wrong, cannot be checked. Also no arrow in MSO with DOC or DOCX. While no arrow in LO, it can be checked. I close this bug as NotOutBug. Could be also WontFix.
