It's normal behaviour.
Dot appear only after some amount of time and/or if there many unread 
notifications.

If you wish immediate notification you may install Notifications Alert
extension - https://extensions.gnome.org/extension/258/notifications-
alert-on-user-menu/

  Unread notifications dot mostly never appears, even when it should

Bug description:
  When a desktop notification comes up and one ignores it it goes into the 
little area for previous notifications next to the calendar. A little dot 
should also appear when you have unread notifications in that section, the dot 
should appear on the right of the date and time at the top.
  However I have found the behaviour of this dot to be inconsistent on Ubuntu 
GNOME 15.10 with GNOME 3.18, sometimes it does show when there are unread 
notifications, 98% of the time it does not though.

  I have also tested Ubuntu GNOME 16.04 with both GNOME 3.18 and 3.19
  and found the issue also to be present there.
