Fixed for me on 0.9.8 network-manager's version. But issue with
disaapeared icon remains. But it is not so important :)

  nm-applet crashes on second or third reconnect

Status in “network-manager-applet” package in Ubuntu:

Bug description:
  Steps to reproduce:

  Step-1: Turn on the mobile-broadband connection by clicking its 
connection-name in the network-manager icon on top.
        EXPECTED: Connection must be turned on and connection-name should not 
change. (In my case it is "Tata Indicom (Photon+) Connection").
        ACTUAL: Connection is turned on, but connection name changes to 
something random (example "CDMA" or "") - See attachment.

  Step-2: Turn off the mobile-broadband and reconnect by clicking on the 
changed connection-name.
        EXPECTED: Connection must be turned and connection-name should not 
change.
        ACTUAL: Connection is turned on, but connection name changes to 
something random (example "CDMA" or ""). Also, if the name was already "" 
(blank), network-manager icon vanishes from top! In that case, I have to do 
"Alt+F2" and type "nm-applet" to restart it to normal.
