Note start-value="1".   Still looking into the bug, figured I should at
least post my preliminary finding.

What applications (OpenOffice/LibreOffice/Microsoft Office/etc) has this
file been edited in?

Title:
  Footnote numbers start at 2 instead of 1

Status in “libreoffice” package in Ubuntu:
  Invalid

Bug description:
  Some how a document has gotten munged so that the footnotes start at 2
  instead of at 1.  Cutting and pasting the text from the document into
  a new, fresh document corrects the problem.  But apparently no amount
  of fiddling can get the count to start at 1 in the old document.
