Unity Launcher window spread stops working randomly

Bug description:
Very often, if multiple windows for an application are open, in unity,
clicking on the launcher icon does not display the window "spread". Only the
last used window is brought to the fore.
All these windows are in the same workspace.
Also, some icons of open programs just did not display in the unity
launcher. They appeared after quite some time.
