This same bug (or at least what looks like this same bug) is still present as of Evince 3.2.1p1 running on OpenBSD 5.1-stable amd64, using the twm window manager.
My test pdf file is US Treasury Form 90-22.1 (revised 1-2012), available from http://www.irs.gov/pub/irs-pdf/f90221.pdf If (after downloading that file, which is a fillable PDF form), I type "evince f90221.pdf" then I can find no way to select one of the radioboxes for item 2. In particular, clicking on or around any of the options has no visible affect on these radioboxes, nor does typing any character I can think of.
