Perl is under a dual GPL/artistic license, and so is this module. You do not have to rename a fork; You could just upload a new package to CPAN and Debian with an increased version number, and a copy of the email where upstream stated that he would incorporate your changes. Yes, it would still be a fork, but it would look like a NMU...
