First, there is a known problem with alsa/kde. In the KDE control centre you need to set sound to ALSA rather than autodetect.
I finally got sound working outside KDE by tweaking alsamixer. Not quite sure what I changed but mpg123 works OK.
The problem seems to be KDE. When I log on it seems to mess up the mixer settings. I've changed KDE not to reload mixer settings on start up and this seems to work OK. However this now means that any mixer changes I do will be lost.
