Just to be sure, I would try "showkey" from a VT. I have seen that at
least some additional keys may send modifiers even when the
corresponding key is not pressed.

Just a notice: you may need to run xmodmap again after disconnecting and
reconnecting a keyboard. Native XKB tools should not suffer from this issue.
