My opinion: - cursor at the top: ok, because this is the perfect way to start editing the reply (bottom-posting without cutting can be worse than top-posting), otherwise you'll have to blacklist mutt+vim :p - cursor at the top with added blank lines: it looks like an invitation to top-posting, should be discouraged.
