Can someone tell me what is causing the following errors?

I installed a new drive a few weeks ago, so I'm thinking that might be
the cause?
This is the second time it's happened today. The first time I lost the
partition and I'll probably lose this one, but I have backups on a
second drive. My cdrom also died a couple of days ago, so things seem
to be going from bad to worse.
