Dunno for sure for ClamAV, but many of the other server services run as dedicated users (for instance, Cyrus, Spamassassin, Postfix...). In fact, one of the things surprised me after arriving to Debian was seeing that each user has its own group. So user "user1" also pertains to group "user1" and not to the whole group of "users". I.e., they have installed one AV in their systems but also perform a file checking using an online AV. The point is that by using an e-mail server AV solution, you are avoiding a high percentage of infections to reach the end-user. And not only viruses, but just spam and other malware if you setup Amavisd-new and SA.
