I was having trouble when reading cards via a card reader. It turned out
that one of contact pins in the card reader which make contacts with the
card were bent out of shape and weren't touching the card at all.
Bending that pin back into shape using a pair of forceps solved the problem.
So, it may be worth it to examine your card reader.
