Of the two flavors of Debian I have used (Debian proper and it's
spin-off Ubuntu), this symptom has popped up on both. Sometimes when I
insert removable media, either disk or USB, the icon that Gnome places
on the desktop winds up on top of an existing icon.

I have used both, Ubuntu and Debian gnome and never saw this happen.
Have you tried with a clean profile?

I have seen this both under Sarge (using an account that used to run KDE but now Gnome, so I can see how that environment might be polluted), and on a machine with a fresh installation of ubuntu with a brand new user account (not sure how much cleaner you can get than that).

Mostly, this seems to crop up after I've used the "Connect to Server" applet to define a nautilus connection to a remote host. The connection icon that appears on the desktop seems to throw off removable media icons, even after I've thrown a couple of other document icons on the desktop too.
