Ehm... don't alter the standard behaviour? I have nice little red arrows indicating the threads, not a single subject line is repeated. I kept my config file from woody, so maybe this is different in sarge or sid? Anyway, there's a nice little manual in /usr/share/doc/mutt with a section on threaded mode, IIRC.
