The important parts for making the wheel work are

Option      "Emulate3Buttons" "off"
Option      "ZAxisMapping" "4 5"

Every thing else should stay to match your physical device.

I set mine up on the first try after following the advice at
http://www.koala.ilog.fr/
(boy was I surprised!)

make sure you're using startx as root. then you can capture the
server messages and find out where it chokes.
