I don't have your card, so I'm stabbing in the dark a little, but it looks to me like you need lines like
alias sound-slot-0 sb in your /etc/modules.conf file. I use a vanilla soundblaster, so I alias mine to "sb". You'll have to do something different there. G'luck....
