I'd say so, but it's so annoying that I'd imagine someone's already
filed a bug about it, long ago.  Certainly have a look through the bug
log, though.

No idea, sorry.  I do seem to vaguely remember that even KDE1.1.2 did
this, though, so kde3 probably has it, too.
