Do you have a link to /etc/init.d/exim in your default runlevel directory? (probably rc2)
Check /etc/rc2.d and see if there's a symlink in there called: S<somenumber>exim. On my computer, it's S20exim. That should point to ../init.d/exim which should then start exim whenever you switch to that runlevel.
