Actually it all described in Linux+NT-Loader HOWTO. If you have
installed HOWTOs then it should in
/usr/share/doc/HOWTO/en-txt/mini/Linux+NT-Loader.txt.gz file.  If you
don't have it you should be able to find this document in Internet.
