Sometimes those little outhouses are actually what's referred to as the "central office." Once upon a time, the CO actually was in the phone company's central office building, but not any more. The phone company can either stuff a whole bunch of analog lines into one digital line, then split it off in a little outhouse. This is typically found in new developments, and the bad news is that it will totally ruin your chances for ADSL. Sometimes, on the other hand, the analog will go all the way to a real phone switch - which can be in one of those outhouses - in that case you're set.
