Assuming your have the 2001 video series, you should have no problems.
You do not need any Chipset etc info. The standard Monitor timings
worked for me also. Note the there are two Diamond Stealth 64 cards
with totally different chipsets.
If you still have problems, I will send you my XF86Config file
