Eric> Help me before I go bald! Usually, behavior of this sort is caused by faulty configuration information in /etc/X11/XF86Config. You claim to have checked the file and confirmed that everything is set up right for your card and monitor, so that would not seem to be the problem. The second thing to check is whether you have the proper X server installed and set as default. The most reliable way to do this under Debian is to simply reinstall the X server package and answer yes when it asks you if you want to set this server as default. Another possibility is that you have multiple XF86Config files on your system and X is looking at the wrong one. 'locate' can be useful in tracking down problems of this nature.
