I don't know what KDE does, but if I change my default X11 cursor theme (by installing e.g. crystalcursors), and turn ui.use_activity_cursor to true, I see the right cursor. I'm inclined to think this is a KDE issue, but I have no idea what package would be responsible. CCing the KDE maintainers for a clue.
