I just upgraded to 3.38.2, restarted gnome-shell and the extension was still here, no error message in the logs. I then disabled it with Tweaks and reenabled it in the same way. It worked fine. Maybe there's a bad interaction with another extension? On my side I have "gnome-shell-extension-timer" and "ubuntu-appindicators" and "system-monitor" among the non-standard extensions.
