The problem has been observed with the /var/spool/mail/[username] file as well as ~/mbox. The problem has been observed with users who are using Apple's mail.app on OS X 10.5, and with users of Outlook Express on Windows XP. I have not seen the problem occur with SquirrelMail, but it's entirely possible that this is because SquirrelMail is used rarely.
