I figured out the cause: perl, by which lsb.pl is written, is inside
/usr.

Roger has expressed this concern before. I did not take it seriously at
that time. Now it bites us..

We should rewrite lsb.pl with POSIX shell or even C, or rely on
functions from insserv (which has a LSB parser) to get rid of this extra
dependency on perl. @Roger, is it easy to borrow the LSB parser from
insserv?

For the moment, you could have an ugly hack to copy a subsystem of perl
over /.
